The concepts are: • Clinical Focus • Patient/Client Advocate • Education and Training • Audit and Research • Consultant Clinical Focus Clinical Nurse Specialist (Gastroenterology) will have a strong patient focus whereby the specialty defines itself as Nursing and subscribes to the overall purpose, functions and ethical standards of Nursing. The clinical practice role may be divided into direct and indirect care. Direct care comprises the assessment, planning, delivery and evaluation of care to the patient, family and/or carer. Indirect care relates to activities that influence and support the provision of direct care. Direct Care Clinical Nurse Specialist (Gastroenterology) will: • Provide a specialist Nursing service for patients with a diagnosis of a Gastroenterologycondition who require support and treatment through the continuum of care. • Undertake comprehensive patient assessment to include physical, psychological, social and spiritual elements of care using best evidence based practice in Gastroenterology care. • Use the outcomes of patient assessment to develop and implement plans of care/case management in conjunction with the multi-disciplinary team (MDT) and the patient, family and/or carer as appropriate. • Monitor and evaluate the patient’s response to treatment and amend the plan of care accordingly in conjunction with the MDT and patient, family and/or carer as appropriate. • Make alterations in the management of patient’s condition in collaboration with the MDT and the patient in line with agreed pathways and policies, procedures, protocols and guidelines (PPPG’s). • Accept appropriate referrals from MDT colleagues. • Co-ordinate investigations, treatment therapies and patient follow-up. • Communicate with patients, family and /or carer as appropriate, to assess patient’s needs and provide relevant support, information, education, advice and counselling as required. • Where appropriate work collaboratively with MDT colleagues across Primary and Secondary Care to provide a seamless service delivery to the patient, family and/or carer as appropriate. • Participate in medication reconciliation taking cognisance of poly-pharmacy and support medical and pharmacy staff with medication reviews and medication management. • Identify and promote specific symptom management strategies as well as the identification of triggers which may cause exacerbation of symptoms. Provide patients with appropriate self-management strategies and escalation pathways. • Manage nurse led Inflammatory Bowel Disease/Gastroenterology clinics with MDT input. • Identify health promotion priorities for the patient, family and/or carer and support patient self-care in line with best evidence. This will include the provision of educational and health promotion material which is comprehensive, easy to understand and meets patient’s needs. Indirect Care Clinical Nurse Specialist Inflammatory Bowel Disease) will: • Identify and agree appropriate referral pathways for patients with Gastroenterology conditions . • Participate in case review with MDT colleagues. • Use a case management approach to patients with complex needs in collaboration with MDT in both Primary and Secondary Care as appropriate. • Take a proactive role in the formulation and provision of evidence based PPPGs relating to Gastroenterology care. • Take a lead role in ensuring the service for patients with a Gastroenterology condition is in line with best practice guidelines and the Safer Better Healthcare Standards (HIQA, 2012). Patient/Client Advocate Clinical Nurse Specialist (Inflammatory Bowel Disease) will: · Communicate, negotiate and represent patient’s family and/or carer values and decisions in relation to their condition in collaboration with MDT colleagues in both Primary and Secondary Care as appropriate. • Develop and support the concept of advocacy, particularly in relation to patient participation in decision making, thereby enabling informed choice of treatment options • Respect and maintain the privacy, dignity and confidentiality of the patient, family and/or carer. • Establish, maintain and improve procedures for collaboration and cooperation between Acute Services, Primary Care and Voluntary Organisations as appropriate. • Proactively challenge any interaction which fails to deliver a quality service to patients. Education & Training : Clinical Nurse Specialist (Inflammatory Bowel Disease) will: • Maintain clinical competence in patient management within Gastroenterology Nursing, keeping up-to-date with relevant research to ensure the implementation of evidence based practice. • Provide the patient, family and/or carer with appropriate information and other supportive interventions to increase their knowledge, skill and confidence in managing their Gastroenterology condition. • Contribute to the design, development and implementation of education programmes and resources for the patient, family and/or carer in relation to Gastroenterology thus empowering them to self- manage their condition. • Provide mentorship and preceptorship for Nursing colleagues as appropriate. • Participate in training programmes for Nursing, MDT colleagues and key stakeholders as appropriate. • Create exchange of learning opportunities within the MDT in relation to evidence based Gastroenterology care delivery through journal clubs, conferences, etc. • Develop and maintain links with Regional Centres for Nursing & Midwifery Education (RCNMEs), the Nursing and Midwifery Planning and Development Units (NMPDUs) and relevant third level Higher Education Institutes (HEIs) in the design, development and delivery of educational programmes in Gastroenterology care. • Be responsible for addressing own continuing professional development needs. Audit & Research: Clinical Nurse Specialist (Inflammatory Bowel Disease) will: • Establish and maintain a register of patients with Gastroenterology conditions within Clinical Nurse Specialist Caseload. • Maintain a record of clinically relevant data aligned to National Key Performance Indicators (KPI’s) as directed and advised by the DoM. • Identify, initiate and conduct Nursing and MDT audit and research projects relevant to the area of practice. • Identify, critically analyse, disseminate and integrate best evidence relating to Gastroenterology care. • Contribute to nursing research on all aspects of Gastroenterology care. • Use the outcomes of audit to improve service provision. • Contribute to service planning and budgetary processes through use of audit data and specialist knowledge. • Monitor, access, utilise and disseminate current relevant research to advise and ensure the provision of informed evidence based practice. Audit expected outcomes including: • Collate data in agreement with the DoN/ADoN which will provide evidence of the effectiveness of Clinical Nurse Specialist (Gastroenterology) interventions undertaken 3 or 4 - Refer to the National Council for the Professional Development of Nursing and Nursing final report - Evaluation of Clinical Nurse and Midwife Specialist and Advanced Nurse and Midwife Practitioner roles in Ireland (SCAPE Report, 2010) and refer to the National KPIs associated with the speciality. They should have a clinical Nursing focus as well as a breakdown of activity - patients seen and treated. • Evaluate audit results and research findings to identify areas for quality improvement in collaboration with nursing management and MDT colleagues (Primary and Secondary Care). Consultant: Clinical Nurse Specialist (Inflammatory Bowel Disease) will: • Provide leadership in clinical practice and act as a resource and role model for Gastroenterology practice. • Generate and contribute to the development of clinical standards and guidelines and support implementation. • Use specialist knowledge to support and enhance generalist nursing practice. • Develop collaborative working relationships with local Gastroenterology Clinical Nurse Specialist (/Registered Advanced Nurse Practitioner/MDT colleagues as appropriate, developing person centred care pathways to promote the integrated model of care delivery. • With the support of the Director of Nursing, attend integrated care planning meetings as required. • Where appropriate develop and maintain relationships with specialist services in voluntary organisations which support patients in the community. • Liaise with other health service providers in the development and on-going delivery of the National Clinical Programme model of care. • Network with other Clinical Nurse Specialist’s in Gastroenterology and in related professional associations. Dunmanway Day Unit CUH is one of nine designated cancer centres of the HSE National Cancer Control Programme (NCCP). Clinical Nurse Manager, Transplant Coordinator, University Hospital
Purpose of the Post The post of CNM 2 has a pivotal role in service planning, co-ordinating, and managing activity and resources within the clinical area. The main responsibilities are: quality assurance, resource management, staffing and staff development, practice development, facilitating communication and professional / clinical leadership and mentorship. Designated Cancer Care Centre CUH is one of the nine designated cancer centres of the HSE National Cancer Care Programme (NCCP) and one of two centres in the southern region. We provide cancer services predominantly to people who live in the counties of Cork, Kerry, South Tipperary, West Waterford and South East Limerick. Our cancer services include tests to diagnose cancer, surgical cancer treatments, haematology oncology services, medical oncology consultation, chemotherapy, and radiotherapy treatments. The Haematology service provides both an in-patient and out-patient service along with a Day Unit Service for malignant and non-malignant Haematology. As well as providing a large general clinical haematology service, the department is a tertiary referral centre for acute leukaemia, and other complex acute adult haematology. There are over 500 inpatient admissions under the Haematology Service annually. More than 90% of these patients have a malignant diagnosis including acute leukaemia, high grade lymphoma and autologous stem cell transplantation. Transplant Coordinator The hospital has a very busy Haematology/Oncology stem cell transplant service. Currently patients are referred to St James’s Hospital and University Hospital Galway for their transplant. The Clinical Nurse Manager 2 (CNM2) - Haematology Oncology Transplant plays a pivotal role in the preparation and follow-up of patients pre and post Stem Cell Transplant. The CNM2 Haematology Oncology Transplant provides a vital link for the patients and the many services which they will need to access during the course of their treatment. The hospital is currently involved in developing plans to allow us to deliver autologous stem cell transplants onsite. Outline of Duties and Responsibilities All administration positions in Cappagh National Orthopaedic Hospital are patient and team focused. The primary responsibility of this position is to facilitate patient care by providing an efficient and effective OPD administration service with a strong focus on patient and customer service as a member of the team. The person appointed will be mainly assigned to the Outpatients Department, and will also provide holiday leave cover for the Medical Records Department and, from time to time, provide relief for the OPD Float position. The role of the Grade III Clerical Officer (OPD) includes, but is not limited to: · This role will rotate within the OPD department to ensure that the person is proficient in all areas of the Department. –The roles of responsibility are as follows: o Opening, sorting, date stamping all post. o Dictation of typing as specified by Supervisor and in accordance with Hospital policy. o Appropriate filing in accordance with Hospital policy. o Welcome patients to the Department and register them ensuring correct patient details are obtained as per hospital policy. o Dealing with telephone and post enquiries where possible and taking clear and concise messages for the clinical staff. Duties The Grade III Clerical Officer (Outpatients Department) will perform the duties of the post as outlined in Part 3 of this Job Description. The person appointed will be required to be flexible in this position and must be prepared to undertake such other duties as assigned from time to time by the Medical Records Officer or Hospital Management or other authorised officer. Such duties can be outside the normal area of work. Patient Care Patient satisfaction must be to the forefront of the post holder’s concern. Every patient is to be treated as an individual and provided with a high-quality service in terms of courtesy, kindness, interest and efficiency. Hours of Work The person appointed will work a five day, 35 hour week but the appointee will attend at such other times as are required for the proper discharge of the duties of the office. The appointee will be required to record their attendance electronically as instructed. In order to comply with Section 33, of the Organisation of Working Time Act 1997 which limits the maximum number of hours an employee is permitted to work in each period, all employees are obliged to notify the Hospital if they engage in any employment in addition to their post with Cappagh National Orthopaedic Hospital. Remuneration Remuneration is in accordance with the salary scale approved by the Department of Health and Children. Current Salary Scale for Clerical Grade III is €29,810 - €46,948 per annum (15 points on scale). Salary is paid by Credit Transfer on the 28th of the month. Periodical increases in salary shall not take effect until the Director of HR/Operations or designated officer certifies that the service of the person concerned has been satisfactory during the year immediately preceding the date of such increments. Probation The successful candidate will be required to serve a probation period of 6 months from the date of taking up employment before being confirmed in the permanent position. During this period, the candidate’s progress will be monitored and Cappagh National Orthopaedic Hospital, may at its discretion, extend the probationary period, in which case the reason for the extension will be made known to the candidate. During the probationary period progress or otherwise will be monitored and at the end of the probationary period the service will (a) be certified as satisfactory and confirmed in writing or (b) if not satisfactory, the probationary period may be extended or certified, with stated reasons, that the service has not been satisfactory in which case the post holder will cease to hold the post. If Cappagh National Orthopaedic Hospital should fail to certify in accordance with (a) or (b) above, the appointment shall be deemed to take effect. In the event of it being determined at any stage during the candidate’s probationary period that his/her employment is unsatisfactory, then his/her employment may be terminated before without recourse to a formal disciplinary process before the probation period expires. Medical Examination The appointment is subject to a successful medical examination and this will be arranged by the hospital. There may be a fee charged for the examination. Superannuation The terms of the Voluntary Hospitals Superannuation Scheme or Single Public Service Pension Scheme will apply to this position (dependent on public sector entrance date). Annual Leave The annual leave entitlement is in accordance with HSE circulars. Sick Leave Granting of payment of salary during illness is discretionary and conditional on the employee complying with the conditions laid down in the sick leave policy and will be in accordance with Department of Health and Children regulations. Termination of Office The employment may be terminated at any time by one months’ notice on either side except where circumstances are dictated by the Minimum Notice and Terms of Employment Act 1973/77. The Hospital Management hopes it will not be necessary to exercise their right under this paragraph. However, in circumstances where Management conclude that the post holder has failed to perform their duties to a standard consistent with their responsibilities, or where their conduct in relation to the post or otherwise would render them unfit to hold the post, the Management right will be exercised. Such records and information are strictly confidential and, unless acting on the instruction of an authorised officer, on no account must information concerning staff, patients or other health service business be divulged or discussed except in the performance of normal duty.
