A panel may be formed as a result of this campaign for Community Healthcare Network GP Lead from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled. Purpose of the Post The Community Healthcare Network (CHN) GP Lead will support the development and maintenance of relationships between GPs, GP practices and Primary Care professionals in order to ensure delivery of high quality, person-centred care to the local population (GP Agreement 2019). The CHN GP Lead will support the Network Manager and CHN Management Team to roll-out clinical programmes; develop innovative solutions and multidisciplinary approaches to challenges within the CHN; and to implement CHN structures and service delivery within CHNs. Children’s Disability Network Team The team provides therapeutic services to children and young people (0 -18) with a range of complex needs arising from a disability and whom are living within the Finglas Network. There will be a strong emphasis on interdisciplinary team working and family centred practice as per the principles of Progressing Disabilities for Children and Young People. To work as a Senior Occupational Therapist in line with team service plans and objectives and within established professional standards, guidelines and policy. To support assessment and interventions in line with principles of Children’s Disability Network Teams and the Disability Act 2005 To work as part of the Childrens Disability Network Team. The team provides therapeutic services to children and young people (0 -18) with a range of complex needs arising from a disability and whom are living within the network. There will be a strong emphasis on interdisciplinary team working and family centred practice as per the principles of Progressing Disabilities for Children and Young People.
