Details of service Sláintecare The Sláintecare Report (2017) and Sláintecare Implementation Strategy (2018) signal a new direction for the delivery of health and social care services in Ireland with the potential to create a far more sustainable, equitable, cost effective system and one that delivers better value for patients and service users. At its core, the strategy focuses on establishing programmes of work to move to a community-led model, providing local populations with access to a comprehensive range of non-acute services at every stage of their lives. Internationally, the strategic repositioning of health services is recognised as a better approach to meet the challenges of escalating demand from an ageing population and the prevalence of chronic diseases, while at the same time ensuring better access to care, addressing inequalities in health and delivering sustainability and best value for population health. The enhanced community care model is currently being implemented with specialist teams for management of chronic disease and care of the older persons and public health nursing will play a significant role in this change model. Senior Physiotherapists in MSK/Older Persons have specific responsibilities in the area of clinical care, support, training, etc., in MSK/ Older Persons. This work with relevant patient cohorts is essential in optimising physical, social and mental health and well-being of persons with such clinical needs and is an integral post in the CHN in provision of holistic care to eligible patients across the life continuum. Purpose of the Post · To work in conjunction with other team members in co-ordinating and developing the Urology service to meet the needs of the population it serves, in line with the objectives of the organisation. · To be responsible for the provision of a high quality physiotherapy service in accordance with standards of professional practice. · To carry out clinical and educational duties as required · To work with the Physiotherapy Manager and CHN team in ensuring the co-ordination, development and delivery of a quality, client centred physiotherapy service across and between networks in the geographical area. · To deliver MSK/Older Persons services in line with best clinical practice. · To develop and implement strategies as part of the MSK/Older Persons service for delivering effective physiotherapy care within a changing environment.
