This is a disadvantaged population with enduring health needs including medical, mental health and addiction issues often with a background of significant trauma The GP will provide medical treatment to patients who present from homeless emergency accommodation. This will be for people who are from a very vulnerable population with significant complex needs and multi-morbidity. S/he will actively participate with other relevant care professionals in planning patient care working as part of the Homeless multidisciplinary team to deliver a co-ordinated approach to eligible client care. The GP will assess patients on presentation, prescribe a course of treatment and monitor that course of treatment.
