The overarching aim of the Strategy is the development of Neuro-Rehabilitation services to improve outcomes for people by providing safe, high quality, person-centred Neuro-Rehabilitation at the lowest appropriate level of complexity. This must be integrated across the care pathway and provided as close to home as possible. These services are to be configured into population based Managed Clinical Rehabilitation Networks (MCRNs). The new model would see the introduction of a multi-tiered system, with access to services based on clinically assessed need. This means that instead of people being referred to a single service, they are now referred to a network of services and can access services within the network based on their need. All referrals to the network are received on a standardised referral form and are processed through a single point of entry through the Network Rehabilitation Coordinator role. Services would work together across organisational boundaries with people moving across the continuum of care seamlessly based on their needs. The MCRN model consists of a multi-tiered system of Neuro-Rehabilitation services and the Community Neuro-Rehabilitation Team (CNRT) provides a critical link in the care pathway by facilitating early discharge and continuity from both acute and post-acute rehabilitation facilities. The Community Neuro Rehabilitation Team is an inter-disciplinary therapy team providing a specialised service for adults with a range of progressive or acquired neurological conditions. This is a multi-professional team including a Consultant in Rehabilitation Medicine, Physiotherapy, Occupational Therapy, Speech and Language Therapy, Psychology, Social Work, Nursing and Administrative staff. This team will provide assessment and intensive therapy to people in both their own homes and in a clinic based setting for up to a period of 12 weeks. This service will provide people with earlier access to specialist rehabilitation close to home. The team provide a person centred service, improving people’s quality of life and working together to support people with neuro-rehabilitative needs. The Operational Team Manager plays a pivotal role in the efficient and effective functioning of the team. The manager holds the operational governance for the team and is the accountable and responsible person for ensuring the delivery of Community Neuro Rehabilitation Team services to the population within the defined catchment. They will co-ordinate the delivery of the services through the identification of clear access and referral pathways to services for those with neuro-rehabilitative need. As this team is part of the Managed Clinical Rehabilitation Network, the newly established Community Neuro-Rehabilitation Team will have a significant role to play with respect to evaluation of the service and contributing to further development of the team and the wider Managed Clinical Rehabilitation Network. Reporting Relationship Reports to the nominated General Manager as the accountable and responsible person for ensuring the delivery of community neuro-rehabilitative services to the population within the defined catchment, working collaboratively with the Heads of Discipline and Consultant in Rehabilitation Medicine. The above reporting relationship is currently under review and may change. Key Working Relationships The day to day key working relationships of the Operational Team Manager, with operational responsibility for the Community Neuro Rehabilitation Team staff, are with the network rehabilitation co-ordinators, Consultant in Rehabilitation Medicine and Heads of Discipline. The Operational Team Manager will work collaboratively with the Neuro-Rehabilitation strategy programme manager on the evaluation and reporting of key service data Purpose of the Post The Operational Team Manager holds the operational governance for the team and will operationally lead the interdisciplinary team who will provide a specialist Neuro-Rehabilitation service. The Operational Team Manager is the accountable and responsible person for ensuring the delivery of high quality, safe, coordinated, Community Neuro-Rehabilitation Team services to the population. This will be provided in accordance with legislative and service delivery frameworks and requirements, within the resources allocated. Using an inter-disciplinary team based model of service delivery, the Operational Team Manager will have full responsibility and accountability for managing specified staff within the Community Neuro Rehabilitation Team. They will provide day to day operational line management for these staff, working collaboratively with the Heads of Discipline who will provide clinical governance and assurance regarding professional practice, quality and standards and will have a line of clinical supervision regarding the practice of each HSE professional within the team. The Operational Team Manager will be responsible for planning, developing, co-ordinating and evaluating the service to meet the needs of the service user and the objectives of the Community Neuro Rehabilitation Team. As this team is part of the Managed Clinical Rehabilitation Network Project, the Operational Team Manager will have a significant role to play with respect to evaluation of the service as part of the MCRN Principal Duties and Responsibilities Operational Management The Operational Team Manager will: · Provide day to day operational management of Community Neuro-Rehabilitation Team staff. · Ensure the efficient, effective and safe delivery of health and personal social services for the Community Neuro Rehabilitation Team, in line with national and local frameworks that comply with national standards, policies and legislation. · Ensure that services are coordinated and delivered to the highest standards within available resources. · Ensure timely access for those referred to Community Neuro Rehabilitation Team services at the level commensurate with their assessed needs in accordance with available resources, and ensure that services are; o person centred; o coordinated and delivered seamlessly; o delivered in accordance with best practice standards and levels of efficiency and fully demonstrate achievement of highest possible outcomes; o Delivered to improve the health and wellbeing of the population. Planning, Performance and Quality Assurance The Operational Team Manager will: · Participate in strategic planning and development of Community Neuro-Rehabilitation Team operational plans in line with National Neuro-Rehabilitation Strategy and guidance from National Neuro-Rehabilitation Steering Group. In conjunction with other Community Neuro Rehabilitation Team’s, and National Neuro-Rehabilitation Programme Manager to ensure a standardised approach across services · In collaboration with Heads of Discipline, ensure performance indicators and targets are monitored and reported through governance arrangements and implement corrective action plans where necessary to ensure targets are met · Manage and respond to complaints and other consumer feedback in accordance with the HSE Comments and Complaints Policy as appropriate. · In collaboration with the Heads of Discipline, ensure the implementation of all the components of a quality assurance system including quality improvement, such as risk management processes and incident investigation analysis, to deliver person safety and a quality service. · Ensure compliance with all standards such as; the National Standards for Safer Better Healthcare, the National standards for the Prevention and Control of Healthcare Associated Infection and develop and implement quality improvement plans as appropriate. · Ensure and demonstrate that all corporate policies, procedures, standards etc. are implemented at local service area level as prescribed by the support / corporate functions. · Collate and provide information to facilitate responses to requests including Parliamentary Questions (PQs), Freedom of Information (FOIs) etc., as required by the Head of Primary Care, Head of Disability and Chief Officer. Finance, HR, ICT and Estates responsibilities The Operational Team Manager will: · With the support of their Line Manager and Finance department, be accountable for the financial management of the Community Neuro-Rehabilitation Team budget in line with the National Financial and Procurement Regulations, ensuring that services are delivered within budget and demonstrate value for money. · In conjunction with ICT and their Line Manager, work to provide ICT systems – clinical & operational – to support the effective & efficient provision of primary care services within the Community Neuro-Rehabilitation Team. · Deal with staffing matters arising in relation to leave, resignations, replacements and all relevant policies in conjunction with Human Resources Department in the CHO. Development of the Service The Operational Team Manager will: · Develop effective integration and collaborative team working relationships with key internal and external stakeholders so that appropriate care pathways can be developed to ensure coordinated services are delivered and maintained in line with population needs. These stakeholders will include: o Head of Primary Care and Disability, o Primary Care and Disability General Manager, o Heads of Discipline, o Rehab Co-ordinators o Primary Care Staff, o Other Community Neuro-Rehabilitation Teams, o Specialist Teams –e.g. Palliative Care Teams, o Other care groups o Referring acute and post-acute hospitals o Voluntary organisations o Service users and service user groups o Private providers, o Staff representative associations, o Community Healthcare Managers. · Liaise with and promote continuous development of these relationships · Liaise with and promote relationship with other contracted providers · Collaborate with Rehabilitation Medicine Consultant and Neuro-Rehabilitation Programme Managers with regard to service developments within the Community Neuro-Rehabilitation Team including implementation of the National Clinical Programmes and standardised processes and care pathways. · Work collaboratively to develop innovative and evidence initiatives and implement responses to identified needs in line with local and national guidelines. · Ensure a multidisciplinary team approach is promoted and adopted within the team · Communicate with all stakeholders, both internal and external to HSE. Leadership & Direction The Operational Team Manager will: · Provide leadership to Community Neuro Rehabilitation Team staff in delivering services consistent with the Standard Operating Procedures of the service to ensure multi-disciplinary approach and coordinated service delivery for the Community Neuro-Rehabilitation Team population. · With the support of Community Neuro Rehabilitation Team staff, provide efficient, effective management and delivery of a team based model of service which is accessible, coordinated and in line with national policy, legislation and delivery framework. · Participate as a member of working groups and other committees as appropriate. · Ensure effective change management arrangements to support and promote the delivery of high quality, safe and reliable healthcare · Provide support, guidance and work with the General Manager to enable the implementation of service organisation and delivery. Governance & Accountability The Operational Team Manager in collaboration with the Head Of Discipline will as appropriate: · Review clinical audits and oversee the implementation of recommendations. · Develop and implement early warning and corrective action planning systems with respect to deviation from planned performance following consultation with key stakeholders and escalate to the General Manager as appropriate. Health & Safety The Operational Team Manager will: · Develop a Site Specific Safety Statement (SSSS) for the Community Neuro-Rehabilitation Team, as applicable, based on the identification of hazards and the assessment of risks, and reviewing/updating same on a regular basis (at least annually) and in the event of any significant change in the work activity or place of work. · Ensure that Occupational Safety and Health (OSH) is coordinated into day-to-day business, providing Systems Of Work (SOW) that are planned, organised, performed, maintained and revised as appropriate, and ensuring that all safety related records are maintained and available for inspection. · Consult and communicate with staff and safety representatives on OSH matters. · Ensure a training needs assessment (TNA) is undertaken for employees, facilitating their attendance at statutory OSH training, and ensuring records are maintained for each employee. · Ensure that all incidents occurring within the relevant department/service are appropriately managed and investigated in accordance with HSE procedures. · Obtain advice from health and safety professionals through the National Health and Safety Function Helpdesk as appropriate. · Review the health and safety performance of the ward/department/service and staff through, respectively, local audit and performance achievement meetings for example. . Other Duties The Operational Team Manager will: · Maintain staff records and collaborate with Heads of Discipline on clinical and professional matters on an individual or collective basis. · Without prejudice to their overarching accountability the Community Neuro-Rehabilitation Team will manage the performance of key personnel within the Community Neuro-Rehabilitation Team through the use of appropriate measurement, benchmarks and metrics. Ensure that the outcomes and KPIs of the individual staff and their collective responsibilities are achieved. · Manage specified resources within the Community Neuro-Rehabilitation Team and develop a workforce plan, reflective of staff performance, leave arrangements, replacement requirements and implementation of the same to ensure appropriate resource allocation to provide adequate services to meet needs of the population. Where resource gaps are identified review requirement for business plan. · Ensure effective system and processes in place for the management and provision of Aids and Appliances, and other consumables and contracted services for the management of persons · To act as spokesperson for the Organisation as required. · To support, promote and actively participate in sustainable energy, water and waste initiatives to create a more sustainable, low carbon and efficient health service. Incremental credit will be applied for recognised relevant service in Ireland and abroad (Department of Health Circular 2/2011).
