Clinical Nurse Specialist (CNS) Acute Hospital Liaison Nurse (Intellectual Disability). or Clinical Nurse Specialist Candidate (cCNS) Acute Hospital Liaison Nurse (Intellectual Disability). Horizons is seeking to recruit a highly motivated and enthusiastic Acute Liaison Nurse. The role of the Acute Care Liaison nurse- ID (CNSp) is to support the implementation of a model of integrated care with Horizons and acute services, enhancing the person supported by Horizons’ healthcare experiences and optimizing service delivery. The Acute Hospital Liaison –I. D will provide good quality and safe health care for people with an intellectual disability and will actively support adults with intellectual disabilities, as well as their carers/families when accessing the acute services. The post-holder will provide expertise and specialist advice on the needs of people with an intellectual disability. They will assist in educational programmes locally and work in partnership with staff/MDT in both Horizons and the acute services to enhance service user experience through positive collaboration. 1. Statutory Registration, Professional Qualifications, Experience, etc (a) Eligible applicants will be those who on the closing date for the competition: (i) Be a registered nurse/midwife on the active Register of Nurses and Midwives held by Nursing and Midwifery Board of Ireland (NMBI) or be eligible to be so registered. AND (ii) Be registered in the division(s) of the Nursing and Midwifery Board of Ireland (NMBI) Register for which the application is being made or be entitled to be so registered.
