Home Sharing Link Social Worker Full time permanent position 35 hour working week Home sharing is a community-based service that provides short breaks to people in Home Sharing families’ homes. The service aims to extend the circles of mainstream supports for the child or adult and family to access community links and social participation. The service is registered with the National Home Share Network (NHSN) and has been developed in line with 2016 HSE National Guidelines. The Role The Home Sharing Link Professionally Qualified Social Worker is committed to the ongoing mission of Horizons to develop different types of short breaks or respite services in an innovative, inclusive and person centred way. The appointee will be knowledgeable in all relevant legislation and will demonstrate an in-depth working knowledge of Models of Services, Advocacy, the Principles of Social Work Practice and CORU standards of proficiency. Skills: The successful candidate must have: • Good judgement, discretion and confidentiality. • The ability to communicate clearly, sensitively, and effectively in both written and verbal forms, and to communicate with family members and work with them for the best outcome for the person supported. • Creative problem solving skills. • The ability to work independently while recognising when to seek support from interdisciplinary colleagues as appropriate. • Patience, flexibility, creativity and eagerness to become involved in a range of activities. • The ability to educate and upskill colleagues and family members in positive health promotion needs • The successful candidate will be required to co-operate in a team environment with a person-centred philosophy, participate in person-centred planning, individual reviews, team meetings, attend and participate in training courses, and actively contribute to team based performance management. Working Hours: 75 hours a fortnight based on a 14 day duty roster covering day and night duty. Flexibility will be required in order to meet service needs. A commitment to providing Senior Person cover within the service is an essential requirement of this post. The Brothers of Charity model of service is based on Personal Outcome Measures requiring employees to be flexible in their working hours to provide a quality service delivery for each individual.
