Location Details:

This is an appointment to the Specialist Diabetes Team - Chronic Disease Management – at Portiuncula University Hospital, Ballinasloe, Co. Galway

There is one initial permanent whole time vacancy available in Portiuncula University Hospital, Ballinasloe, Co. Galway

A panel may be formed as a result of this campaign for Clinical Nurse Specialist (Diabetes), PUH from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled.

The post holder will work as part of an Specialist Consultant led multidisciplinary team delivering evidence based care to adults aged 18 and over with diabetes, referred for chronic disease (diabetes Type 1 and 2) management in the hospital in-patient and out-patient setting. The team are part of the integrated chronic disease management Diabetes team working across primary and hospital care.

Whilst based in the hospital a part of the CNS Diabetes Integrated Care may be carried out “offsite” at out patient clinics established in the community specialist ambulatory care hub for chronic diseases.

The allocation of time spent in the hub will be determined by local service needs. This proportion of time allocated may be subject to restructuring in the future to facilitate the re-organisation of services in line with the development and expansion of clinical models of acute and community services (Chronic Diseases and Enhanced Community Care).
