Clinical Specialist Occupational Therapist Frailty Intervention Team

The Clinical Specialist Occupational Therapist will have responsibility for provision of specialised inpatient, +/- outpatient (where applicable) Occupational Therapy Service for the frail population presenting to ED as part of the multidisciplinary FIT team in the Mater Misericordiae University Hospital. This service is part of the wider Care of the Older Person Occupational Therapy service which comprises the Frailty Intervention Team (FIT), the Acute Medical Assessment Unit, the Specialist Geriatric Ward, COTOP Day Hospital and specialist off site post-acute care service. There is a strong MDT focus across the services with the use of the Comprehensive Geriatric Assessment from ED throughout, with a home first ethos.

Clinical input may be required into the wider Care of the Older Person services and any other service that may be required from time to time.

We are one of the largest FIT teams in the country with Consultant Geriatrician Lead, 1 Clinical Specialist OT, 1 Senior OT, 1 Clinical Specialist Physiotherapist, 1 Senior Physiotherapist, 1 Clinical Specialist Dietitian, 1 Clinical Specialist SLT, 1 Senior MSW, 2 Registered Advanced Nurse Practitioner, 1 Clinical Nurse Specialist, 1 Pharmacist and 1 Geriatric Registrar. We are based in ED and work closely with all our ED colleagues. We have excellent links with our Integrated Care Team for Older Persons who take patients directly from our service to home. We have access to direct admission to two offsite rehab units for patients identified in ED with rehab needs and goals.

We have presented our data at both National and International conferences and have a strong research and audit focus across the team.

To identify and assess the frail population presenting to ED as part of the multi-disciplinary FIT team in the Mater Misericordiae University Hospital in accordance with the mission, values and strategic plan of the organisation.

To act as senior decision-maker, with recommendations regarding HSCP intervention requirements & need for hospital admission, where appropriate.

To determine discharge plan and appropriate follow-up arrangements with relevant services.

To alternate team leader role within FIT team, providing leadership and direction to the FIT team activities.

To participate in the management of the occupational therapy service as a member of the senior occupational therapy team in co-operation with the occupational therapy manager.

To analyse service demands and develop the FIT team with other HSCP members in line with evolving requirements.

To analyse service demands across the COTOP OT service and link with OT management regarding same.

To undertake highly skilled and specialised assessment and intervention as part of the occupational therapy process. These include but are not limited to assessment and intervention related to:

· Activities of daily living

· Cognitive perception

· Postural management and positioning

· Sensorimotor function of the upper limb

· Delirium

· Management of Falls

· Discharge Planning.
