Clinical Nurse Specialist, Falls Prevention

The Falls Prevention Clinical Nurse Specialist will manage a delegated caseload of patients who have fallen, are at risk of falls or who have a fear of falling, delivering individualised and personalised direct and indirect patient care. Patients are referred to the Falls Prevention Clinical Nurse Specialist for specific reasons i.e. if they have had a fall with or without harm as an inpatient within the MMUH, patients who have had a fall at home, patients who have had recurrent falls and patients who are at risk of falling. The Clinical Nurse Specialist in Falls Prevention will provide care for these patients providing falls prevention education, assessing for individual risk for extrinsic, intrinsic, environmental factors, identifying medications that could increase a person’s risk of falls/polypharmacy and ensuring a plan of care for patients identified at risk of falls is in place during their in-patient stay and on their discharge. As the Mater establishes itself as a Major Trauma Centre over the next 10-15 years the caseload of the Falls Prevention Nurse Specialist will respond to this new exciting service as the prevention of future injury is a key priority for older adults following trauma.
