Each Sector has a population of 50,000 approx. At present Galway/Roscommon M.H.S is extending and developing the Community Services in relation to Homebased Treatment Teams, Crisis Intervention Teams, Rehabilitation Teams and specialist teams in line with the needs of the population and “Sharing the Vision”. · Areas covered by the services include the urban and rural centres of, Co. Galway (including Galway City) and Co. Roscommon (including Roscommon town). Galway/Roscommon Mental Health Services consists of inpatient and community mental health services strategically located in Co Galway and Co Roscommon. · Acute Adult Inpatient Units are located in Galway City (attached to University Hospital Galway) and Roscommon town, (attached to Roscommon University Hospital). An inpatient unit for Psychiatry of Later Life is situated in Ballinasloe. There are 5 approved centres in Galway and Roscommon Mental Health Services licensed and regulated under the Mental Health Act 2001. · Community Mental Health Teams are located in each of the sectors providing mental Health care and treatment in Day centres, Day Hospitals, Residential Units and the individuals who avail of our service at own home. Service Mission, Vision and Values Eating disorders have the highest mortality and morbidity of all of the mental health conditions (Arcelus, 2011), and it is estimated that they will affect between 1 to 4% of the population at some point in their lives (NIMH, 2016). They are caused by a combination of genetic, biological and psychosocial factors and occur across gender, age, cultural, ethnic and socioeconomic groupings. Although not common eating disorders result in very high psychosocial and economic cost to individuals, families, healthcare and society when not treated or treated ineffectively (Butterfly Foundation 2012 and 2015). The service mission, vision and values of the Galway Roscommon Mental Health service include: • To improve the clinical outcomes and recovery of people with eating disorders in Galway and Roscommon though the provision of evidence based practices by trained, skilled staff. • To reduce the comorbidity and mortality associated with eating disorders through enhanced clinical risk management and early intervention. • To remain patient centred by including service user feedback and input in the development of our eating disorder services. • To support enhanced screening and early recognition of eating disorders. • To provide timely access to consultation for referrers. • To reduce service user internal and external waiting times to initial assessment and treatment. • To develop a stepped care model of service delivery based on different patient needs. • To establish formal pathways between services that will enable more seamless transitions of people with eating disorders when it is needed. • To evaluate the effectiveness of our services across clinical, activity and service user experience outcomes. • To reduce the significant long term health, economic and psychosocial cost of eating disorders through early intervention. • To reduce the duration of illness and promote recovery through the provision of evidence based effective interventions. • To reduce dependence on inpatient admission (numbers / length of stay) where clinically appropriate, though the provision of alternative less restrictive options for intensive treatment such as day and group programmes. • • To reduce relapse rates though all of the above measures. Reporting Arrangements Is professionally accountable to the Assistant Director of Nursing and Director of Nursing Is clinically accountable to the supervising Consultant/Clinical Lead/ GP Clinical Indemnity Indemnity arrangements for CHO2 Mental Health Services are provided by the State Claims Agency’s Clinical Indemnity Scheme; the Advanced Nurse Practitioner, Candidate (Eating Disorders-General Adult, Mental Health Services) is indemnified under the above scheme. The RANP will work within defined and agreed scope of practice and in accordance with approved policies, protocols, procedures and guidelines (PPPGs) and established clinical supervision arrangements. The structure, process and outcome of clinical supervision must be explicit. The cANP Eating Disorders – General Adult Mental Health Services maintains a record of clinical supervision in his/her professional practice portfolio. Purpose of the Post The main purpose of the post is to develop the job description and supporting documentation under the direction of the Health Care Provider’s Advanced Practice Stakeholder Governance Group, to enable the individual nurse to meet the NMBI Criteria for Registration as an Advanced Nurse Practitioner as set out in Advanced Practice (Nursing) Standards and Requirements (NMBI, 2017) . The individual will undertake the academic preparation and develop the clinical and leadership skills, competencies and knowledge required to meet the criteria to be registered as a RANP with NMBI. The scope of the cANP role must reflect the incremental development of expertise and as such, the cANP cannot deliver care as an autonomous practitioner. The individual will complete and submit the necessary documentation for registration as an RANP with NMBI. The value of the nursing contribution as a distinct profession must be safeguarded and articulated in the development of new services led by advanced nurse practitioners, complementing rather than replacing current services delivered by doctors (NMBI 2017 p.9). The overall purpose of the post is to provide safe, timely, evidenced based nurse-led care to patients at an advanced nursing level. This involves undertaking and documenting a complete episode of patient care (assess, diagnose, plan, treat and discharge patients) according to collaboratively agreed protocols and scope of practice in the clinical setting; demonstrating advanced clinical and theoretical knowledge, critical thinking, clinical leadership and decision making skills. The advanced practice role demonstrates a high degree of knowledge, skill and experience that is applied within the nurse-patient/client relationship to achieve optimal outcomes through critical analysis, problem solving and accurate decision-making (NMBI, 2017). Central to this is the provision of quality care, a safe environment and processes for patients by the use of evidence based clinical guidelines that address patient expectations, promote wellness and evaluate care given. The role will provide clinical leadership and professional scholarship in order to develop nursing practice and health policy at local, regional and national level. The role will contribute to nursing research to shape and advance nursing practice, education and health care policy at local, national and international levels. Principle Duties and Responsibilities The post holder’s practice is based on developing a higher level of capability across the six domains of competences as defined by Bord Altranais agus Cnáimhseachais na hÉireann Advanced Practice (Nursing) Standards and Requirements (NMBI 2017) The domains are: Appointment as an employee of the Health Service Executive is governed by the Health Act 2004 and the Public Service Management (Recruitment and Appointment) Act 2004. Purpose of the Post The Staff Nurse (Mental Health) will assess, plan, implement and evaluate care to the highest professional and ethical standards utilising the model of nursing care practiced in that care setting.
