Location of the Post:

The current vacancy is permanent, whole time based in Block B, Civic Centre, Main Street Bray, Co. Wicklow/ Bloc B, Lárionad Cathrach, An tSráid Mhór, Bré, Co. Chill Mhantáin

A supplementary panel may be created for Grade VII Case Manager Liaison Officer (Supplementary campaign)within Community Healthcare East (CH East) from which permanent and specified purpose vacancies of full or part-time duration may be filled.

Please note that there are existing panels in place for Grade VII Case Manager within Community Healthcare East (CH East). The existing panels will take precedence over the supplementary panel created through this campaign for any future posts that arise.

Purpose of the Post:

Disability Case Manager can provide holistic, supportive care tailored to the unique needs of individuals with disabilities, enhancing their overall well-being and opportunities for a fulfilling life.

Case management is a system for the planning, arranging and subsequent co-ordination and delivery of services using individually tailored care plans, having a person-centred approach and a multi-disciplinary focus. The initial purpose of this post is to appraise options and arrange appropriate service provision for persons with a disability in consultation with Disability Team, in accordance with assessed needs and having regard to the available resources.

This job specification is intended as a guide to the general range of duties and is intended to be neither definitive nor restrictive. The range of duties will be subject to periodic review with the post holder. It is planned to undertake a formal review of the post not later than 1 year following appointment and the structure of the post may change as a result. Post holders will be consulted as part ofthis development. Successful candidates will be accepting the post on the basis of this review and possible restructuring at a future date.
